Survey Details Screen
This is the new screen we are adding to the prototype.
The screen will be linked from Health Center > Feedback > Clinic Satisfaction survey.
When user completes the survey and tap on the button, we show alert message “Thank you!” and a dismiss button, when tapped on it it should take user back to the Feedback tab screen.
